Format

Note

Substantial restructuring from 2010: revision codes revert to R/U, birth time added at positions 25-28, paternity acknowledged added at 152, unrevised education fields dropped, father's education added, prior terminations added, WIC and pre-pregnancy cigarettes added, infections block added, maternal morbidity block added, APGAR-10 added, and a large new block of fields (524-568) covering BMI, inter-pregnancy intervals, maternal height/weight, and set order. Reporting flags restructured to 4-level codes.

Identical structure to 2011 except father's combined age unrevised (positions 184-185) is dropped to filler.
